If a certificate for overnight accommodation treatment is requested by an insurance company, etc., the Prefectural Infectious Disease Control Division will issue a certificate for overnight accommodation treatment.Please enclose the following two items and mail them to the address below.
If the form is designated by the insurance company, etc., please enclose it.
The certificate will be returned by specified recorded mail.
If the documents are incomplete, we may call you for confirmation.
Please note that it usually takes about three weeks to issue the certificate.

* Shiga Prefecture cannot issue a certificate of accommodation and medical treatment, so please use a certificate of medical treatment from My HER-SYS, a document that shows the results of a PCR test performed at a medical institution, a medical statement, etc.[Persons subject to notification of infection]

- Those who need hospitalization
- Those who are at risk of aggravation and need administration of new corona therapeutic drugs, or those who are at risk of aggravation and need new oxygen administration due to new corona infection
- Pregnant women

(2) Persons who are not eligible for reporting the infection cannot use either the My HER-SYS medical treatment certificate nor the Shiga Prefecture accommodation medical treatment certificate.
