In Japan, “specific medical checkups” (Tokutei Kenshin) are available to those who are enrolled in health insurance from the age 40 to 74. The health insurance company you are enrolled in will subsidize the cost of a medical checkup once a year.★ For those who are employed
It is mandatory for the employers to have the employees undergo a medical checkup once a year.If the company offers you a medical checkup, be sure to take it. The employer will be responsible for the cost of the examination items specified by the law.
★Dependents who are enrolled in Kyokai Kenpo
Every April dependents over the age of 40 will receive a notice on specific medical checkup.Enclosed are medical checkup tickets and pamphlets. Please make an appointment in advance at the hospital or venue where you would like to receive a specific medical checkup. If you have a medical checkup ticket, 7,150 yen will be deducted from the total cost. There are also venues where you can receive medical checkups free of charge, so be sure to undergo a medical checkup when you receive a notice.
★Those who are enrolled in National Health Insurance (Kokumin Kenkou Hoken)

Workers who are not covered by social insurance are short-term part-timers and other workers whose contract period is less than two months. Since employment during the trial period is considered to be employment based on the assumption that the employee will be officially hired at a later date, the employee must be enrolled in social insurance from the beginning of the trial period.

From October 2022, the national system was revised, and medical expenses for a visit to a large hospital without a referral letter became more expensive.This amount is not covered by health insurance and are not included in high-cost medical care expenses. Those between the ages 35-74 working at a company insured with Kyoukai Kenpo are able to receive a detailed health checkup for approximately 7,000 yen. Family members enrolled in the same insurance (aged 40 - 74) may receive simple checkups such as blood tests from 0 yen to approximately 1,500 yen (fees vary depending on method of examination).

A hokensho is an insurance card to be presented when visiting a hospital. It is issued by a company or municipal office. When you present your insurance card at a hospital in Japan, you can be treated at 30% of the total medical expense determined by national standards. ● Payments at the hospital are only 30% of the total cost for both types. Benefits for high medical bills, child delivery, and death are also the same. Process of enrollment and withdrawal of health insurance
- Employment at a company
- The company will undergo procedures required for enrollment into social insurance, however, it is the responsibility of the individual to withdraw from national health insurance. Please visit your city hall of residence for withdrawal procedures to prevent double billing.
- Leaving a company
- When you leave your company and lose qualifications for social insurance, return your insurance card to your company as your card is no longer valid. You will have to visit your city hall to undergo procedures for enrollment into national health insurance in order to receive a health insurance card. Please check for other options, such as continuing optional social insurance or becoming a dependent of a family member.
